| Mandatory Health (Medical) Insurance
Requirement
The university requires, as a condition of registration,
that all international students have adequate medical insurance
coverage during all periods of enrollment at UCLA.
UCLA Undergraduate Student Health Insurance Plan (SHIP)
UCLA offers a low-cost, comprehensive plan, the Student
Health Insurance Plan (SHIP), available when you pay your
university fees for each quarter.
You may enroll dependents in SHIP by obtaining a Dependent
Enrollment Form from the Arthur
Ashe Student Health and Wellness Center. This form must
be submitted within ten days of enrolling yourself in the
plan. Although UCLA does not require medical insurance for
dependents, we strongly recommend it due to the high cost
of health care in the U.S. The SHIP Insurance Office has staff
available to answer questions regarding the plan. Call (310)

SHIP Fees
The SHIP fee is already included in your registration
fee assessment each term.
Opting Out of SHIP
If you wish to acquire another authorized medical plan you
must submit a completed SHIP Waiver Form before the
registration fee payment deadline each term.
To qualify as "adequate" private medical insurance
plans must meet all of the following minimum requirements:
- Provide a minimum of $100,000 in lifetime benefits; have
PP0/HMO facilities located within 25 miles of UCLA.
- Cover at least 75% of your medical expenses; have a deductible
of $500 or less and a co-pay of 20% or less
- Be issued by a U.S. carrier
- Include medical evacuation and repatriation benefits for
those on J-1 and J-2 visas.
The following plans are NOT acceptable and do not fulfill
the university health insurance requirement:
- Travel insurance plans of any kind
- Any plan purchased outside the U.S. and/or from a non-U.S.
company
- Reimbursement arrangements or vouchers, including those
from home, government, and consulates in the U.S.
